| Tax/Fees/Cancellation Notes: | VACATION RENTAL RULES AND REGULATIONS Please be sure to read this agreement in its entirety to see if any additional fees apply to your reservation- This is a NON SMOKING room.- SECURITY/DAMAGE DEPOSIT- If a credit card is not given at time of booking, a security/damage deposit of $200 is required. The deposit is NOT applied toward rent; however, it is fully refundable within (14) days of departure, provided the following provisions are met. 1.) No damage is done to room or its contents, beyond normal wear and All debris, rubbish and discards are placed at disposal area of the hotel? 3.) FALSIFIED RESERVATIONS - Any reservation obtained under false pretense will be subject to forfeiture of advance payment, deposit and/or rental money, and the party will not be permitted to check in. PAYMENT - An Initial Deposit equal to one (1) night of rent and tax will be collected to confirm your reservation, and will be applied to the final balance. Payment in full is required 30 days prior to arrival. If you are not paying by credit card, please make payments in the form of traveler’s checks, bank money orders, cashiers checks or personal checks payable to: Hawaii 5-0 Vacation Rental, Inc. and send to 1288 Ala Moana Blvd. #4A, Honolulu, HI. 96814 CREDITS OF WITHHELD DEPOSITS- Any deposit withheld due to cancellation may be applied for a future booking in that same unit. Credits are not transferrable to other parties, and must be used within 365 days from the original reservation start date. Credit is made in a dollar amount, so that any reservation made with credit will be booked at the current going rate. This rate and tax percentage may exceed the credit of the withheld deposit and tenant is responsible for paying the difference between these two. TRAVEL INSURANCE: To offer you the best services possible, we strictly adhere to our cancellation policy. However, to help protect your vacation investment, Hawaii 5-O is able to include travel insurance through Travel Guard as part of your reservation. Travel insurance is typically at a cost equal to 7% of the rent, tax and cleaning total of your reservation. For more information on Travel Guard please request a flyer, or call (Refer to product number 008573P1). If you choose to decline travel insurance please notify your rental agent.- WRITTEN EXCEPTIONS - Any exceptions to the above mentioned policies must be approved in writing in advance.- LOCAL TAX - Renter is responsible for 13.962% local hotel room tax in addition to daily room rate.
